MMI Form
Click to see the latest ATO MMI form, so that you
can print it or download it. If you are sending the Payment
Summary files to the ATO on a disk (CD, diskette or USB) you will need
to send the completed MMI form with the disk.
- If you are sending files by ECI, you do not need the MMI form.
- If you are sending one file only, just send the first page of the MMI form only.
- If you are sending more than one file on the
disk, send two pages of MMI form. The first page asks for the
name and ABN of just one payer - write the name and ABN of the payer
for which enter File Sequence number as 1. On the second page
write the name and ABN of the payers corresponding to all the other
files. It is possible that there are several files for the one
payer, for example if the weekly and monthly pays are in separate
employer groups - if so write the name and ABN twice.
